Screen Check Test Ph Vaginal is an in vitro diagnostic medical device CE 0123. Read the warnings and instructions for use carefully. Authorization of XX / XX / XXXX

• Remove the test from the sealed pouch
• Test immediately after opening the package for best results
• Place the test on a clean, flat surface
• Place the tip of the swab soaked in vaginal secretion on the pH area of the test for at least 5 times, making sure that enough secretions are applied to the pH area of the test
• Read the results immediately when the measurement area is still wet
• Compare the color of the pH zone with the reference on the package
